Scabies and crab lice (also known as pubic lice) are common skin infestations caused by tiny parasites that live on or under the skin. They cause intense itching, irritation, and visible signs like rashes or red bumps, often in areas like the wrists, waistline, groin, and underarms.

Scabies is caused by mites that burrow into the skin to lay eggs. This triggers an allergic reaction, leading to itching and irritation. Crab lice attach themselves to coarse body hair—most often around the groin, but they can also be found in armpits, beards, or even eyelashes.

The body’s immune system reacts to the mites or lice and their waste products, causing inflammation, itching (especially at night), and sometimes secondary infections from scratching.

Common Signs and Symptoms
For scabies: intense itching, fine red lines (burrows) on the skin, small red spots or blisters.
For crab lice: itching in the affected area, visible lice or eggs (nits) near the base of hairs, and blue spots or red bumps from bites.

Risk increases with close physical or sexual contact with an infected person, sharing clothing, towels, or bedding, and in crowded living conditions or where hygiene is poor.

Medications

Medications for Scabies and Crab Lice

Effective treatment requires targeted medications to eliminate the parasites and relieve symptoms. Some are available directly from pharmacies, while others require a prescription, especially for severe or resistant cases.

Pharmacy-Only Treatments (P Medicines):

Permethrin 5% Cream – The first-line treatment for scabies and crab lice. Available without a prescription, but only under a pharmacist’s supervision. It kills mites and lice by disrupting their nervous system. Applied to the whole body (excluding head in adults unless otherwise directed) and washed off after 8–12 hours.

Malathion 0.5% Lotion – Another pharmacy-only option, mainly used for lice infestations. It destroys both lice and their eggs. Applied to affected areas and washed off after a set period, usually 8–12 hours.

Prescription-Only Medications (POMs):

Ivermectin Tablets – An oral antiparasitic prescribed in severe, resistant, or crusted scabies. It’s also used in institutional outbreaks or where topical treatments are unsuitable. Not typically a first-line treatment but highly effective when required.

Benzyl Benzoate Lotion / Sulfur Ointment – These are occasionally prescribed when first-line treatments are not suitable, though they may cause skin irritation or be less cosmetically acceptable.

Why Proper Treatment Matters

Scabies and crab lice are highly contagious. Prompt and effective treatment helps prevent ongoing symptoms, complications from scratching, and transmission to others. If pharmacy treatments are ineffective, or symptoms persist, prescription options may be necessary.
